New Vendor Request Form
  •  If your organization requires new vendor paperwork or registration in an online database, you must submit this request by email to: [email protected]
  • Please include detailed instructions if applicable. These requests are typically completed in less than 5 days.
 
Insurance and Licensing Requests
  • If your organization requires specific insurance declarations, including certificates of general liability, worker’s compensation, subrogation waivers, etc., you must submit this request by email to [email protected]
  • Please include detailed instructions if applicable. These requests are completed in order they are received and may take more than 5 days, depending on the complexity of the requirements.

Billing Questions

When will I receive my invoice?

Invoices are automatically generated within one hour of your registration and emailed to the Billing Contact on file. Please ensure your email address and the Billing contact’s email address are correct when registering.

When is my invoice due?

Payment is typically due 45 days from the invoice date. Additionally, payment is NOT due before the start of the training. If your agency requires additional time to pay or other accommodations don’t hesitate to contact Accounts Receivable at [email protected]. If your payment is in process and you receive an overdue notice, please disregard the notice.

How do I make changes to my invoice?

We are here to help! If you received an invoice in error, need a revised invoice, or need your invoice sent to someone else for payment, contact Kristen in Accounts Receivable at: [email protected]

Who do I send vendor registration paperwork to?

My agency requires a signed Purchase Order for payment. Who should I contact?

You can mail, fax, or email Purchase Orders to Accounts Receivable.

What is your cancellation policy?

You may transfer your registration to another officer at any time, just email us the replacement’s name and email address. We ask that cancellations be provided at least 48 hours before the start of the class. However, we understand that our students have unpredictable schedules so please email [email protected] or call 888-579-7796 and let us know how we can help.

Training Questions

Is there a dress code? What do I bring?

All materials are provided except for pens and paper for note taking. Please follow your Agency’s Dress Code policy for outside training when attending class (we want students to be comfortable and prefer causal dress).

What time does class start and end?

Unless otherwise stated on the class announcement flyer, class begins at 8:30 am and ends at approximately 4:30 pm.

What is the class address?

Classes are hosted at various places around the country. Please check the Upcoming Training page on Blue to Gold’s website to locate your training address: www.bluetogold.com/calendar

I was registered twice. What should I do?

Please email [email protected] or call 888-579-7796 as soon as possible to delete the duplicate registration. This allows us to ensure the maximum number of students can enroll in a class and prevents invoice errors.

Are there restrictions to transfer my registration?

No, there are no restrictions and you can transfer a registration at any time, including the morning of the class. Please email [email protected] or call 888-579-7796 as soon as possible to transfer the registration. If last minute, have the replacement show up to class and inform the instructor about the transfer so that the new student can be added to the roster.

Who do I contact to cancel a registration?

Please email [email protected] or call 888-579-7796 as soon as possible to cancel a registration. This allows us to ensure the maximum number of students can enroll in a class and prevents invoice errors.

Can I register without providing the student’s information now?

Yes, no problem. Just register using the name John or Jane Doe to hold the number of seats you need. When you have finalized your attendee list, email this information to [email protected] or call 888-579-7796. Please send us the information no later than the Thursday before the training date so that the instructor is provided an accurate roster.

I have not received my certificate, who should I contact?

First, please check your spam folder for an email from Blue to Gold, email is [email protected]. If you do not have it, email [email protected] and you will get your certificate(s) right away.
